COVID-19 vaccines were administered to three people by mistake in Washington, military officials said.

The people went to get “another vaccine” at the Joint Base Lewis-McChord home of I Corps and 62nd Airlift Wing in Tacoma but ended up with a Pfizer COVID-19 shot instead, Col. Joey J. Sullinger said in a statement.

It’s unclear when the shots were given or what vaccines they intended to receive.

“Positive corrective action has been taken at this vaccination site to prevent such errors from happening again. Army medical professionals are notifying and ensuring the health and welfare of those affected,” Sullinger said.

The incident is being investigated.
